CHE Trinity Health Partners with Sharecare To Accelerate Consumer and Patient Engagement

Chicago, October 23, 2014 – CHE Trinity Health, one of the nation’s leading health care systems, and Sharecare, the health and wellness engagement platform, today announced at the CHE Trinity Health Clinical Summit that they are partnering to provide consumers and patients with a personalized, localized engagement platform across the 20 states of the CHE Trinity Health system.

The partnership between CHE Trinity Health and Sharecare will support individual and community health outside of the clinical environment in ways that are convenient for consumers. In addition to engaging consumers in their wellness with informative and compelling digital content, CHE Trinity Health’s regional health systems will have new avenues for patient acquisition and online physician profiles.

“CHE Trinity Health is building a people-centered health system, and our partnership with Sharecare provides an innovative way for our regional health ministries to connect people with their health and wellness in their daily lives,” said Scott Nordlund, Executive Vice President for Growth, Strategy and Innovation Development at CHE Trinity Health. “People expect and deserve health care where and when they choose, and our partnership with Sharecare is a big, people-centered step forward.”

The patient-engagement platform created with CHE Trinity Health will enable people to create a central health profile and gain personalized access to:

  • The leading scientifically-based health risk assessment, the RealAge® Test, taken by more than 40 million people and validated in peer-reviewed journal PLOS ONE;
  • Individualized programs to promote general wellness in areas such as weight loss, smoking cessation, stress management and diabetes prevention;
  • Cutting-edge digital health integrations, including the ability to connect health trackers and sensors, and AskMD, Sharecare’s award-winning health consultation app;
  • Tools to locate a CHE Trinity Health physician, schedule an appointment and communicate with the physician; and
  • Content to help patients prepare for surgery, therapy or other clinical services, and aid their recovery afterwards.

“Connected health is helping people live happier, healthier lives – not because of the technology alone, but because it enables the health system to start a conversation and build a relationship directly with the patient outside of the exam room,” said Jeff Arnold, Chairman and CEO of Sharecare. “The combination of Sharecare’s proven engagement platform with the clinical excellence at CHE Trinity stands to accelerate healthcare innovation, and improve the health and wellness of those who need it most.”

In addition to powering the health system’s patient-facing engagement platform, Sharecare will give its audience of engaged consumers the option to connect with CHE Trinity Health’s physicians and clinical services based on their specific medical need and personal profile. Sharecare and CHE Trinity Health will also collaborate on social media and local community initiatives aimed at helping consumers become more proactive about managing their overall health.
