Join our case study review of CareLinx by Sharecare’s transitional care program that reduced readmissions and other hospitalization events by 21%, positively impacting quality measures related to risk-adjusted utilization and producing cost savings.
Professional caregivers provided in-home support and became the “eyes and ears” of the health plan-helping to improve member satisfaction post-discharge, promote clinical program adoption, and support better post-acute quality of life for members. In addition, this program was used to identify and close care gaps as well as to create opportunities to connect members with the full array of benefits and services available to them through the plan.
